Standing Watch
Outward — the people who stopped callingEverything First Watch does, across multiple lines, plus the outbound half: following up what went quiet and re-contacting people who drifted. This is the tier that reaches out.
Silent Watch
Inward — every line you ownFor businesses that cannot or will not do outbound. Instead of reaching further out, it goes deeper in: every line under one standard, an escalation tree that proves the handoff connected, and a response commitment in writing. It contacts nobody, ever. That is the product, not a limitation.

A transfer that rings out is worse than no transfer, because the caller believed something was happening. From Standing Watch upward, handoff is a tree rather than a single destination: named people in a defined order, confirmation that the handoff actually connected, and a defined next step when it doesn't.
In a medical practice this is not a convenience. A caller who needs a clinician during business hours is routed to the on-call nurse practitioner, with the same confirmation and the same fallback. A practice that needs that routing wants Standing Watch or Silent Watch, not First Watch, and we will say so rather than sell you the cheaper tier and let you find out.
What the agent does not do is decide who needs a clinician. It does not triage, it gives no clinical advice of any kind, and it routes on what the caller asks for and on your written rules — never on a judgement about how serious something sounds. If a caller describes an emergency it tells them to call 911 first, and it says so before anything else.
